Before you fill this out: This is a real intake, not a marketing form. This form asks for four things only: what to call you, an email address, a category, and a short description. Do not put anyone's legal name, address, phone number, medical or court detail in the description — that comes later, off this form, if I take the case. Intake goes over HTTPS to a managed database that encrypts data at rest and is filed under a short reference rather than your legal name. It is not end-to-end encrypted: I can read it, and the hosting and database providers' systems process it in plaintext. If I decline, withdraw, or never hear back from you, the record is deleted from the active database 30 days after that decision. Full terms are in the Privacy Notice.

I confirm that anyone represented on my side of the working (self, children, dependents in my care) has given consent as far as they are able to, or that I hold legal authority to represent them. I understand that no working here will be aimed at someone whose only "offense" is not returning my feelings.

I understand that Dana Batista guarantees the integrity of the craft (the ritual described in the protocol is the ritual cast), and does not and cannot guarantee specific outcomes. I understand no working replaces mundane action — legal, medical, or safety-related — that my situation may require.

The $75 intake fee is due when I open your case — non-refundable in all cases, whether I take the working or decline it. It pays for my time reading and divining on the case. I will send a Stripe invoice by email within one business day of receiving this form; divination begins once it is paid. The ritual fee is a separate charge, quoted only if I take the case.
